Is the alleged father willing to do a paternity test with us?

Do you want to test for paternity before the child is born or after birth?

Is the mother of the child currently pregnant? If yes, how many weeks gestation?

If the mother of the child is currently pregnant, is the pregnancy known to be multiples (twins, triplets, etc)?

If the mother of the child is currently pregnant, was the pregnancy via surrogacy or In Vitro Fertilization (IVF)?

Did the mother of the child have a bone marrow transplant or a recent blood transfusion?

Does the mother of the child have a medical condition known to alter DNA, such as cancer, chromosomal disorders, monogenic disorders, etc?

Does the child have any known fetal chromosomal abnormalities?

Do any of the test participants exhibit any form of chimerism?

Once your intake form is received, we will call you for a free initial consultation to confirm your testing needs.

Click the “Book now” button to schedule an appointment date/time to have the specimens collected.

Once your appointment request is received, we’ll email you an invoice with a payment link. Appointment is confirmed when payment is received in full.

Step 5

At your scheduled appointment time, show up to 708 E. Elms Rd, Killeen, TX 76542 to have your samples collected (unless a mobile collection location has been coordinated). Ensure to bring photo ID. If children don’t have a photo ID, their birth certificate can be used as an alternate.
